Extra help with Part D costs
Medicare's Extra Help program (also called the low-income subsidy) can pay for most or all of your prescription drug costs if you qualify.
What is Extra Help?
Extra Help is a federal program that helps people with limited income and resources pay for Medicare Part D prescription drug coverage. If you qualify, Extra Help can pay for:
- Part D monthly premiums
- Annual deductibles
- Copays and coinsurance at the pharmacy
You also won't pay a late enrollment penalty while receiving Extra Help, even if you enrolled late.
Extra Help is worth an estimated $5,000+ per year
The average value of Extra Help benefits is significant. If you think you might qualify, it's worth applying — there's no cost to apply, and you can't be penalized for being found ineligible.
Who qualifies?
You may qualify for Extra Help if you have Medicare Part A or Part B and meet income and resource limits. As of 2026:
- Income limit — generally below 150% of the federal poverty level
- Resource limit — limited savings, investments, and real estate (not counting your home or car)
Some people qualify automatically and don't need to apply:
- People with full Medicaid benefits
- People enrolled in a Medicare Savings Program
- People receiving Supplemental Security Income (SSI)
How to apply
There are three ways to apply for Extra Help:
- Online — apply at ssa.gov/prescriptionhelp
- By phone — call Social Security at 1-800-772-1213 (TTY 1-800-325-0778)
- In person — visit your local Social Security office

State pharmaceutical assistance programs
Many states offer additional programs that help pay for prescription drugs, beyond what Extra Help provides. These State Pharmaceutical Assistance Programs (SPAPs) vary by state and may cover costs that Extra Help does not.
